Transition Aged Care Program (TACP)

Enrich Living Services offers short term, goal orientated support to people returning home after hospital to help you get back into home life fast after a hospital episode. The Transitional Aged Care Program (or TACP for short) is available in suburbs in Sydney south west.

TACP offers a range of services that aim to help you recover your mobility and independence at home. It is available for up to 12 weeks following hospital admission to assist you until you are fully recovered. In some cases, where needed, it can be extended a further 6 weeks with approval from an Aged Care Assessment Team.

TACP is a flexible program, with services available 7 days a week, and can be personalised to your individual recovery needs.

Some of the services to choose from include:

  • Nursing support
  • Personal care
  • Allied health, for example occupational therap
  • Help around the house with domestic tasks
  • Transport

Enrich Living Services is funded to provide TACP from Bankstown, Bowral, Campden and Campbelltown, Fairfield and Liverpool hospitals.

Accessing TACP

The cost of the program is subsidised by the government, but depending on your pension status, you may be asked to pay a small contribution. You will be assessed by the Aged Care Assessment team in the hospital.
